ongratulations! You looked through our donor database and decided on a sperm donor. You also decided with your doctor if you will have IUI, ICI or IVF. The most difficult part of the process lies behind you! Here are the final steps on buying the donor sperm:
1. Download the patient forms for purchasing sperm. Get the forms. Please note that there are three forms in the package.
2. Fill out patient forms on your computer and email or fax completed forms to our Clinical Director at 619-265-1429. (Instructions on how to fill-in your information on the forms here. If you do not have Adobe, click here.)
3. Call our office for payment information of the vials and fees of transportation (if applicable). Tell us how many vials you would like to purchase. and whether you need storage for parts of your purchase and shipping. Please note that if you purchase 2-5 vials, storage of unused vials for one year are complementary. If you purchase 6 or more vials, your storage fees are waived for 3 years. Fees are as follows:

Vial Buy-Back and Exchange Program
We also offer a vial buy-back option and vial exchange program of donor sperm purchased and stored at FCC facilities. Please inquire for details.
